PT Day HHA/CNA AM and PM Shift

Job Summary
We are seeking a compassionate and dedicated Home Health Aide to provide essential support to individuals in the facility. The ideal candidate will assist clients with daily living activities, ensuring their comfort and well-being while promoting independence. This role requires a strong commitment to patient care and adherence to health and safety regulations.

Hours: Part Time - 6 am to 11 am and 2 pm to 10 pm. Every other weekend/Holiday.

Responsibilities

• Provide personal care services including bathing, grooming, and dressing assistance.
• Assist with medication administration as directed by healthcare professionals.
• Observe and report changes in the patient's condition or behavior to the supervising nurse or physician.
• Support clients with mobility needs, including lifting and transferring as necessary.
• Prepare nutritious meals according to dietary restrictions and preferences.
• Maintain a clean and safe environment for clients, adhering to HIPAA regulations at all times.
• Engage clients in social activities and provide companionship to enhance their quality of life.
• Document care provided and maintain accurate records of services rendered.

Experience

• Previous experience in caregiving or home health care is preferred but not required.
• Familiarity with senior care practices and home community care is an advantage.
• Strong observational skills with the ability to recognize changes in patients’ conditions.
• Excellent communication skills, both verbal and written, are essential for effective interaction with clients and healthcare professionals.
