Meadow Grove Transitional Care is seeking an experienced and compassionate Wound Care Nurse (RN or LPN) to support our residents through high-quality wound assessment, treatment, and prevention. This role is ideal for a clinically strong nurse who enjoys focused wound care and collaborative teamwork.

Assess, treat, and monitor acute and chronic wounds including pressure injuries, surgical wounds, diabetic ulcers, and skin integrity concerns

Perform routine wound rounds, measurements, and documentation

Develop and update individualized wound care plans in collaboration with physicians and interdisciplinary teams

Track wound healing progress and report changes in condition promptly

Educate nursing staff on wound prevention, treatment protocols, and best practices

Ensure compliance with facility policies, infection control standards, and state/federal regulations

Maintain accurate and timely documentation in the electronic medical record (EMR) PCC
