Responsibilities:
Provide direct patient care to residents, including administering medications, assessing vital signs, and performing various medical procedures
Develop and implement individualized care plans for each resident, in collaboration with the healthcare team
Monitor and report resident progress, identifying any changes or concerns to the nursing staff
Maintain accurate and up-to-date medical records, adhering to HIPAA standards and regulations
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
Participate in resident care conferences and other interdisciplinary meetings
Ensure a clean and safe environment for residents, adhering to infection control and safety protocols
Provide emotional support and comfort to residents, families, and caregivers
Requirements:
Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Ohio
