Full time position for an RN or LPN with previous MDS experience. Full benefits with an on-call rotation.
Responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans for our residents that meet Federal and State guidelines, thereby providing optimal care. Facilitates patient care conferences and is responsible to effectively manage care plans in the best interest of the patient and the facility. Promotes and participates in ongoing education to maintain and enhance knowledge of current / new regulations.
JOB FUNCTIONS: (This list may not include all duties assigned.)
1. Provides guidance to charge nurses about the care plan process. Keeps abreast of changes in regulation and policy and interprets changes for all team members.
- Regularly makes rounds and/or attends nursing morning report and updates care plan with any changes. Monitors residents for changes in condition, adjusts the plan of care and resident’s response to that care, and notates possible significant changes to MDS.
- Monitors and ensures completion of 7-day look-back documentation by nursing staff within prescribed MDS timeframe. Communicates appropriate issues to Director of Nursing Services and Nursing Supervisors.
- Facilitates patient care conferences and coordinates the activities of the disciplines of nursing, social services, rehabilitation, dietary, activities and restorative nursing. Incorporates information from all healthcare team members in developing the plan of care.
- Reviews all resident incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.
- Identifies discrepancies that may be present in the plan of care and assists the team to analyze and resolve differences in opinions.
- Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
- Sets best ARD in conjunction with therapy to ensure highest reimbursement rate possible.
- Establishes MDS schedule and distributes to all RTCP members. Notifies families and/or residents of scheduled RTCP & sends invitation letter to responsible party. Completes MDS, CAAs, updates care plan and runs the RTCP meetings. Prints updated care plan for unit.
- Reviews summary status reports and generates a summary of triggered problems and rationale.
- Evaluates learning needs of staff, regarding the MDS process and assists in the on-going education of staff related to the MDS and supporting documentation.
- Assists in maintaining compliance with the standards of nursing practice.
- Weekly monitors all residents at RTCP meetings for potential restraint reduction, skin issues and fall interventions.
- Submits MDS to ECS and transmits MDS to CMS and Equip timely.
- Shares in call rotation.
- Performs other duties as assigned.
WORKING CONDITIONS:
: Requires frequent rolling greater than 80 pounds, often combined with bending and turning. Must be able to push/pull a loaded medication cart over carpeted floor for extended distances for significant periods of time. Requires the ability to respond, direct and intervene during stat calls throughout building in a timely manner. Requires ability to walk up and down stairs. Requires corrected vision and hearing within normal limits. Requires use of medical equipment, office equipment such as computers, telephones, fax machines and copy machines. Requires use of medical equipment in the clinical setting. Requires eye/hand coordination and manual dexterity. Requires frequent prolonged standing, sitting, walking, and the ability to perform physical labor for an eight-hour day.
: Work is primarily performed in a long-term care facility. Contact with staff, residents, families.
EMPLOYMENT REQUIREMENTS:

Knowledge of nursing theory and practice. Knowledge concerning federal and state regulations pertaining to assessment and planning. Knowledge of the MDS (Minimum
Data Set) process, understanding the purpose and effective management of care to ensure financial health of the facility. Competency in providing guidance to the nursing facility about the care plan process. Knowledge of interpretive guidelines used by surveyors to determine compliance. Ability to assess, plan, implement, evaluate, and document care and instructions. Ability to analyze data from patient outcomes and adjust care plans to modify resident care. Demonstrates personal integrity and empathy and fosters such in others.
Graduate of an accredited Registered Nursing program. Bachelor’s Degree in nursing.
Current Connecticut State License in good standing as a Registered Nurse.
Three years experience in clinical nursing in acute care or long-term care. Supervisory experience in a long-term care setting preferred. Minimum of two years’ geriatric experience.
