What are the responsibilities and job description for the Part Time Case Manager position at McKinley Hall Inc?
PRIMARY RESPONSIBILITIES
- Plan, coordinate, and carry out activities in conjunction with, or on behalf of, the client.
- Assists the treatment team in identifying problems, needs, and development of individualized treatment plans.
- Become fully familiar with the Ohio Recovery Housing Standards and ensure the agency is in compliance (applicable for Recovery Housing Case Managers).
ADDITIONAL RESPONSIBILITIES
- Establish a case management relationship with clients and maintain that relationship regardless of service environment to ensure continuity of care and provision of service.
- Maintain waiting list of clients referred to the Residential and Intensive Outpatient or Women's Programming. Keeps regular contact with clients awaiting admission and referrals. Schedule appointments for clients going into those Programs.
- Provide orientation of treatment program for clients; assist clients with necessary paperwork and contact individuals involved with client's treatment.
- Establish relationships with other agencies in order to provide necessary services for our client population.
- Coordinate the interaction with service agencies, legal authorities, and all concerned parties with the client, and act as the client's advocate in obtaining services.
- Advocate in the community for clients and for the treatment program.
- Assist clients to obtain eligible benefits, i.e., SSI, GA, or Medicaid.
- Ensure medical needs for clients are met, working with the Nurse and Medical Director.
- Help clients obtain educational/vocational services by making appointments, and following up as necessary.
- Assists clients in developing means of transportation. Provides transportation rarely, and only as appropriate given client level of need.
- Participate in treatment team staffing and assist clinical staff in treatment planning. Prioritize those needs which affect client's successful completion of treatment.
- Assist clients to develop non-treatment aftercare goals and ways of achieving them, i.e., housing.
- Performs off-site assessment when requested.
- Provide back-up direct treatment services as necessary, facilitating groups and individual therapy.
- Inform supervisor of important developments in case management.
- Maintain funding source standards by meeting productivity budgeted requirements.
- Comply with Agency, State, and accrediting body requirements by documenting and completing all required paperwork.
- Serve on special committees, task groups or projects as directed by supervisor.
- Represent the Agency in the Community.
CASE MANAGER
- Other duties as requested by supervisor.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
- Associate’s Degree in Social Work or Related Field
- 2 or more years of Case Management experience.
- Knowledge of Ohio Recovery Housing rules and regulations preferred.
- Meets OMHAS Licensure/Certification Standards for Treatment Providers
KNOWLEDGE, SKILLS AND ABILITIES NECESSARY
- Computer Literacy
- Ability to define problems, collect data, establish facts and draw complex conclusions
- Ability to work as a member of a team with excellent communication and listening skills
- Mobility among various job sites
- Good driving record and insurable with agency insurance without an increase in premium
- Ability to work evenings and/or weekends

- Participate in annual CPR and First Aid training

UNUSUAL WORKING CONDITIONS
- Exposure to clients with alcohol and other drug related problems.
- Potential for exposure to Hepatitis B, Tuberculosis, and other blood-borne or airborne pathogen diseases.
- Requires transporting clients in an agency vehicle
