Summary
Under broad supervision, provides a broad range of social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/ or serve as a liaison for social services. May perform case management duties. Performs lead worker functions.
Responsibilities
· Assist individuals in identifying vocational interests and skills and obtaining and maintaining employment.
· Provides vocational assessment to consumers who are interested in work and educate them on the role of the vocational technician on the ACT team. Responsible for teaching vocational and pre-vocational skills such as time management, resume building, job-seeking, interviewing, and on-the-job support.
· Maintains a caseload of 10-12 consumers as assigned: acting as the main point of contact for those consumers, ensuring a scheduled MD appointment that month, and assisting in arranging transportation/planning.
· At least 80% of services are provided to consumers in the community. Attends average 4-weekly treatment team meetings.
· Available for after-hours (crisis phone) coverage as assigned.
· Perform related duties assigned by the Supervisor and maintain compliance with all company policies and procedures.
Minimum Qualifications
· Bachelor's degree in a social service-related field from an accredited college or university AND One year of experience in vocational specialist field OR High school diploma or GED AND Three years of experience as a vocational specialist.
· Note: Some positions require a valid Georgia’s Driver’s License.

· Ability to assess crisis situations and provide crisis interventions.
Physical Requirements
· Must be able to lift and carry up to 25/50 lbs.
