Duties
- Assist residents or clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe changes in health status to inform care plans
- Administer medications following HIPAA guidelines and care protocols under supervision
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Maintain cleanliness of living spaces through regular cleaning and sanitation practices
- Use specialized equipment such as Hoyer lifts or resident’s
to ensure safe transfers and mobility assistance - Follow detailed care plans designed by healthcare professionals to ensure consistent, high-quality patient support
- Document patient observations, progress notes, and incident reports accurately and timely
- Work collaboratively within home health, hospice, assisted living, or group home environments to deliver comprehensive care
Qualifications
- Proven experience in home & community care, assisted living, nursing homes, or group homes is highly desirable
- Knowledge of patient monitoring techniques including vital signs assessment and Traumatic Brian injuries
- Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina for long shifts is essential
- Strong observational skills for patient safety and health status updates
- Excellent communication skills to collaborate effectively with team members, residents, families, and healthcare providers
- Compassionate attitude with a genuine desire to improve the lives of others through direct support and caregiving excellence
