The applicant must possess an in-depth knowledge and understanding of health promotion and disease prevention principles, and knowledge of community resources. The applicant must also possess knowledge and skills needed to use computer applications, resources to assess individual health status, create individualized health profiles, and engage residents of African descent and provide assistance in the development of plans to prevent avoidable health risk. Also, the applicant must possess an understanding of cultural competency in providing effective prevention services.
Job Summary:
The Community Health Worker (CHWs) will be responsible for helping County residents of African descent understand their individual health risks, how to improve their health status and the potential consequences of uncontrolled health-related risks. Also, CHWs will work collaboratively with residents of African descent to adopt healthy lifestyles and behaviors. Tasks and activities will include initial assessment of health status, development of a short and long-term plan to improve health, and to counseling about how to promote, maintain and improve the health status of the resident and family. CHWs will offer informal counseling about pregnancy, diabetes management, and control, cardiovascular health, STI/HIV/AIDS, cancer prevention tips and strategies based on the most recent evidence. CHWs will advocate for individuals and community health needs, and provide health screenings, encourage the development and monitoring of compliance with detailed plans negotiated with residents to improve their overall health status.
Key Functions/ Responsibilities
· Conduct interviews and assessments of individual health status
· Administer and explain the results of physical and emotional health screenings
· Provide information about blood pressure control, diabetes management, stress reduction, pre and post-natal health care, and weight management
· Provide assistance and support needed to create and establish a mobile personal health portal
· Teach residents how to populate, store, update and maintain personal health information
· Conduct HIV/AIDS tests and provide pre and posttest counseling
· Explain diagnostic lab test results
· Help residents set personal goals, attend health related appointments, and adhere to medical advice.
· Provide referrals for services to community agencies as appropriate
· Provide instructions and guidance about how to use computer applications that are designed to record and track wellness related progress and outcomes
· Conduct presentations for community organizations, agencies, and other stakeholders
· Communicate information on vital health information
· Conduct community outreach and establish and strengthen community collaborations
· Support data management protocols
· Attend regular staff meetings, training and other meetings as required which may occur evenings and/or weekends
· Other duties as assigned
Minimum Qualifications
· BA degree or an equivalency degree from a recognize institutions of higher learning
· Successful completion of a Community Health Worker formal training program
· Experience in working in multicultural setting
· Knowledge of medical terminology
· Knowledge and understanding of cultural competency
· Basic computer knowledge and skills
· Ability and willingness to provide support and encouragement to residents who are interested in improving and maintain their health
