QUALIFICATIONS:
- Bachelor's degree in Social Work or a related field (or equivalent combination of education and experience) with at least two years of case management experience, preferably in nonprofit, public sector, or residential settings.
- Experience supporting diverse older adult populations, including individuals with disabilities and complex medical, mental health, substance use, housing, or domestic violence-related challenges.
- Strong collaboration, advocacy, and interpersonal skills with the ability to work effectively with community agencies, service providers, and multidisciplinary teams while managing an independent caseload.
- Proficient in case documentation, written communication, analytical thinking, and Microsoft Office applications, including Outlook and Windows-based systems.
RESPONSIBILITIES
- Provide comprehensive case management and social service coordination, including crisis intervention, client advocacy, counseling, referrals, and connecting residents with community resources to support independent living.
- Develop and maintain client service plans and documentation while collaborating with residents, families, property management, and community agencies to coordinate care.
- Foster a positive residential community by mediating conflicts, supporting tenant success in independent living, and serving as a resource to property management staff.
- Organize resident engagement activities, assist with Food Bank distribution, participate in meetings and training, and support additional program initiatives as needed.
