Job Description and Qualifications for
Aftercare Specialist
Job Title: Aftercare Specialist (ACS)
Qualifications:
Education: An Aftercare Specialist must possess a High School Diploma or equivalent. Bachelor’s degree preferred but not required.
Experience: This position requires a minimum of one year experience working with adolescents in a child care agency, child placement agency or educational setting, or relevant life experience such as parenting, or other experiences related to child care. Preferred experience includes that which involves providing direct service to at-risk adolescents.
Additional Requirements: The Aftercare Specialist must complete thirty-two hours of training during the first year of employment and twenty-four hours per year thereafter.
Reporting Responsibilities:
The Aftercare Specialist reports to the Program Director.
Responsibilities:
The Aftercare Specialist is primarily responsible for providing outreach and follow-up services to youth and their families. In addition to providing direct services to the youth and their families, the responsibilities of the Aftercare Specialist should also include, but are not limited to:
1. Attending, participating, and facilitating Treatment Team meetings and trainings
2. Attending and participating in joint inter-agency meetings
3. Facilitating and supervising group activities, one-on-one encounters, and timeouts
4. Fulfilling all certifications and trainings requirements
5. Maintaining a safe environment by being knowledgeable about safety policies and procedures
6. Communicating appropriately and effectively with colleagues and referral sources regarding youth behavior, needs, and program progress
7. Developing aftercare plans
8. Working as a YDW on floor when coverage is needed
9. Monitoring youth’s extra-curricular activities
10. Being an active member of the Administrative Team
11. Facilitating the re-integration of program graduates into the community
12. Meeting with program graduates as outlined in their aftercare plans
13. Supervising and participating in activities with the youth when it does not interfere with outreach and follow-up responsibilities
14. Providing consistent on-call support
15. Providing assistance to youth and their families in accessing ancillary services
16. Completing weekly school visits with aftercare youth and communicating with school employees regarding behaviors and academic progress
17. Completing court review reports and weekly progress feedback sheets
18. Attending and testifying at youth court hearings
19. Planning, organizing, and facilitating graduation ceremonies – including invitations
20. Maintaining case notes for all youth contacts
21. Taking, typing, and filing Treatment Team meeting minutes
22. Submitting case notes and phone contacts monthly to supervisor
23. Completing surveillance plans with input from the Treatment Team
24. Completing random home visits and phone calls for surveillance purposes
25. Recording mileage and turning in mileage log with the first of the month timesheet
26. Conducting random drug screens when appropriate
27. Reviewing weekly progress feedback sheets with youth
28. Meet with supervisor minimum of three times per week
29. Avoid all forms of misconduct (i.e. theft, sexual harassment, forgery, abuse, gossiping, sleeping, excessive absenteeism, excessive tardiness, drugs, alcohol, job duties, criminal convictions, fighting, unauthorized visitors)
30. Completing other duties as assigned by the Program Director
Aftercare Specialist is expected to maintain professional standing in all relationships and in the community when representing the Isabella County Day Treatment Program.
All staff are expected to display initiative in approaching their supervisor with ideas related to program structure and programming. Staff are expected to demonstrate commitment to the program by:
1. Exhibiting a willingness to work flexible hours when required
2. Learning new material and skills to improve youth care
3. Attending and satisfactorily completing all trainings
4. Acting as appropriate role models in relations with youth
5. Representing the agency in a professional manner in the community
6. Demonstrating appropriate behaviors and actions, both in the program and in the community
In addition, staff with ongoing duties shall be of good moral character, emotionally stable, and of sufficient health, ability, experience, and education to perform the duties assigned. Staff convicted of any crime related to abuse, neglect, alcohol, drug use, theft, or violence is required to report his/her supervisor immediately. Staff must also report any changes in his/her driving record.
