Job Title: School Adjustment Counselor Reports to: Clinical Director Status:Exempt Salary Range: $65,000-$75,000 Primary Responsibility: The School Adjustment Counselor provides social-emotional support and counseling services to students, helping them overcome personal, social, and behavioral challenges that interfere with academic success. This role collaborates with teachers, administrators, families, and community resources to promote student well-being and a positive school climate. Essential Duties: In all areas, perform other related duties as required and/or assigned. ·Work collaboratively with the Education Director, Clinical Director, and Dean of Students to support the academic, behavioral, and social-emotional needs of students. ·Develop and implement intervention plans to support students’ mental health and academic success. ·Maintain a clinical caseload of day students, with potential involvement with residential education students. ·Provide individual and group clinical support within the school setting using a trauma-informed care lens. ·Coach and consult with teachers and instructional assistants regarding students’ clinical needs, behavior regulation, and trauma-responsive strategies. ·Partner with families of day students, including conducting home visits as clinically indicated. ·Collaborate with outside providers and agencies (therapists, DCF, outpatient clinicians, placements) to ensure continuity of care. ·Develop, implement, and monitor individualized behavior support plans ·Contribute to the development of IEP goals and objectives and complete quarterly progress reports ·Attend and actively participate in IEP meetings, case conferences, and multidisciplinary team meetings ·Collaborate with medical providers and attend medication reviews as needed ·Support transition planning for students entering or exiting the program ·Support students during school activities, including field trips and community-based programming ·Participate in Extended School Year (ESY) and summer programming ·Conduct risk assessments and crisis intervention when necessary. ·Maintain accurate and confidential records in compliance with school policies and state regulations. ·Participate in IEP meetings and contribute to the development of social-emotional goals for students with special needs. ·Support staff with de-escalation strategies and post-crisis processing. ·Track and analyze behavioral and clinical data to inform interventions and measure student progress. ·Participate in program development and continuous quality improvement initiatives Education: • Master’s degree in Social Work, Counseling, Psychology, or related field, (in process acceptable) Qualifications: •State licensure (pending acceptable) or certification as a School Adjustment Counselor or equivalent. •Experience working with children and adolescents in an educational setting. •Strong communication, problem-solving, and crisis management skills. •Knowledge of trauma-informed practices and cultural competency. Certifications: •Maintain active certification in First Aid, CPR, CPI Non-violent Crisis Intervention and attend all other required trainings •Must have a valid driver’s license Skills/Abilities: •Communicates effectively for administration and management, case management services, community, and inter-agency relations •Demonstrates leadership skills, maturity, and ability to deal effectively with emotionally demanding and stressful situations. Physical/Mental Requirements: •Must demonstrate good judgment in decision making and determination of information that should be communicated to staff and other supervisors •Must fully accept demonstrate and represent Nazareth’s stated mission, vision, and values in daily work •Ability to read, write, and speak English; Bilingual capacity in English/Spanish an asset •Must be able to communicate effectively by telephone and in person. Must be able to record clear messages and keep accurate records. •Must be proficient in Word, Excel, PowerPoint, and can quickly learn any needed new computer skills •Prepares thorough, accurate and understandable communications and reports in a timely manner •Ability to provide direct coaching and supervision of employees •Must be able to communicate effectively by telephone or in person. •Must have manual dexterity and motor skills to grasp and lift objects of up to 25 lbs. •Must be able to bend, assist in lifting a person, twist and sit on floor for implementation of daily schedule. •Must be able to physically escort and physically restrain individuals. •Must be able to bend and lift up 25 lbs. to purchase and/or access program supplies. •Must be able to climb for entry and mobility throughout the buildings, which may have several levels of stairs. •Must demonstrate maturity and ability to deal effectively with emotionally demanding and stressful situations. •Must be able to maintain and exercise confidentiality at all times. •Must be able to travel to various sites to attend meetings, conferences, and seminars. •Continues to achieve a satisfactory competency and performance evaluation annual Procedure: Date: Approved by Board of Directors: 12/2025 Distributed to Staff: 12/2025
