LVN / LPN – Home Health & Hospice

We’re looking for a compassionate and dependable LVN/LPN who enjoys building meaningful relationships and providing hands-on care. In this role, you’ll support patients in their homes—helping them recover, manage conditions, or receive comfort-focused hospice care.

We’re looking for a compassionate and dependable LVN/LPN who enjoys building meaningful relationships and providing hands-on care. In this role, you’ll support patients in their homes—helping them recover, manage conditions, or receive comfort-focused hospice care.
What You’ll Do
- Provide direct patient care including treatments, monitoring, and support
- Assist with care plans for home health and/or hospice patients
- Observe and report changes in patient condition to the RN/physician
- Educate patients and families on basic care and daily needs
- Collaborate with the care team to ensure quality and continuity of care
- Complete accurate and timely documentation
- One-on-one patient care in a home setting

- More independence compared to facility-based roles
- Active LVN/LPN license (state of practice)
- 1 year of clinical experience (home health, hospice, or bedside preferred)
- Strong attention to detail and communication skills
- Ability to work independently and follow direction from RN/clinical team
- Valid driver’s license and reliable transportation
