We're looking for a compassionate and dependable Licensed Practical Nurse (LPN) who enjoys providing one-on-one patient care in a home health setting. What You'll Do
- Provide skilled nursing care under the direction of a Registered Nurse and physician
- Administer medications and treatments as ordered
- Monitor patient conditions and report changes to the supervising RN
- Reinforce patient and caregiver education on medications, treatments, and disease management
- Assist with developing and following individualized plans of care
- Complete timely and accurate clinical documentation
- Collaborate with physicians, caregivers, and the interdisciplinary care team

What You Bring
- Active LPN/LVN license (state of practice)
- Previous home health, hospice, long-term care, or clinical nursing experience preferred
- Strong clinical, communication, and organizational skills
- Ability to work independently while managing multiple patients
- Valid driver's license and reliable transportation
