The Hospice RN Case Manager is a licensed Registered Nurse responsible for overseeing the provision of high-quality hospice care to patients and families. Qualifications:
- Current, unrestricted Registered Nurse (RN) license in the state of practice.
- Minimum of 2–3 years of clinical nursing experience; hospice or palliative care experience strongly preferred.
- Prior supervisory or leadership experience preferred.
- Knowledge of hospice philosophy, end-of-life care, and applicable regulations.
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent interpersonal, communication, and leadership abilities.
- Valid driver’s license and reliable transportation (for home visits, as required).
