Responsibilities
- Coordinate services for individuals served in the CILA program.
- Develop, review, and monitor Personal Plans and Implementation Strategies.
- Communicate with guardians, families, ISSA, day programs, medical providers, and staff.
- Train and guide DSP staff on individual goals, supports, rights, and documentation.
- Monitor individual progress, health, safety, behaviors, and service needs.
- Complete and review required documentation, progress notes, reports, and follow-ups.
- Assist with surveys, audits, incident follow-up, and regulatory compliance.
- Promote individual rights, dignity, independence, and community involvement.
Experience
- Proven experience working with individuals with developmental disabilities in assisted living or group home settings.
- Extensive background in residential care environments.
- Demonstrated leadership in supervising healthcare teams.
- Knowledge of health regulation policies applicable to residential facilities.
- Skilled in developing care plans that address complex needs such as medication management and behavioral support strategies.
- Familiarity with social work principles and experience in home & community-based services enhances your ability to deliver comprehensive resident-centered care.
Qualifications
- Must meet Illinois QIDP requirements.
- Bachelor’s degree in a human services field or other qualifying credential.
- At least one year of experience working with individuals with developmental disabilities.
- CILA or residential experience preferred.
- Strong communication, documentation, organization, and follow-up skills.
- Must pass required background checks and complete required trainings.
