Responsibilities

  • Coordinate services for individuals served in the CILA program.
  • Develop, review, and monitor Personal Plans and Implementation Strategies.
  • Communicate with guardians, families, ISSA, day programs, medical providers, and staff.
  • Train and guide DSP staff on individual goals, supports, rights, and documentation.
  • Monitor individual progress, health, safety, behaviors, and service needs.
  • Complete and review required documentation, progress notes, reports, and follow-ups.
  • Assist with surveys, audits, incident follow-up, and regulatory compliance.
  • Promote individual rights, dignity, independence, and community involvement.

Experience

  • Proven experience working with individuals with developmental disabilities in assisted living or group home settings.
  • Extensive background in residential care environments.
  • Demonstrated leadership in supervising healthcare teams.
  • Knowledge of health regulation policies applicable to residential facilities.
  • Skilled in developing care plans that address complex needs such as medication management and behavioral support strategies.
  • Familiarity with social work principles and experience in home & community-based services enhances your ability to deliver comprehensive resident-centered care.

Qualifications

  • Must meet Illinois QIDP requirements.
  • Bachelor’s degree in a human services field or other qualifying credential.
  • At least one year of experience working with individuals with developmental disabilities.
  • CILA or residential experience preferred.
  • Strong communication, documentation, organization, and follow-up skills.
  • Must pass required background checks and complete required trainings.

Pay: From $52,080.06 per year
