Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health status by observing vital signs and overall condition; report changes promptly to healthcare professionals
- Support medication administration following care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals and assist with feeding as needed, ensuring dietary restrictions are followed precisely
- Operate patient transfer equipment such as Hoyer lifts and manual handling techniques to ensure safe movement
- Maintain a clean and safe environment through regular cleaning duties and adherence to infection control protocols
- Document care activities accurately in client records, including patient observations, progress notes, and incident reports
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable
- Demonstrated knowledge of dementia care, Alzheimer’s care, memory care, and hospice care practices
- Familiarity with patient monitoring techniques such as vital signs measurement and medication administration
- Hands-on experience with patient transfer methods including Hoyer lifts and other mobility aids
- Knowledge of care planning processes and assisting with ADLs in various settings such as geriatrics or developmental disabilities support
- Certification in CPR and first aid is preferred; ongoing training in manual handling, HIPAA compliance, and patient personal care is advantageous
