Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Deliver skilled nursing services in accordance with physician orders and care plans
- Initiate and update care plans in collaboration with physicians and care teams
- Monitor and assess patient conditions, including physiological and psychosocial changes
- Respond to changes appropriately, reporting to physicians or supervisors as needed
- Administer medications and treatments as prescribed, monitoring and documenting effectiveness
- Collect, analyze, and interpret patient data to identify actual or potential nursing needs
- Document care, observations, and patient progress accurately and timely
- Collaborate with the healthcare team to ensure continuity and quality of care
- Identify needs related to patient education, care coordination, goal setting, and discharge planning
Qualifications
- Active Massachusetts nursing license:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN)
- Current Basic Life Support (BLS) certification
- Current TB test, PPD, or chest X-ray (as required)
- Current health certificate (per contract or state regulation)
- Minimum of one year of professional nursing experience preferred
