Your Role
As a Homecare Nurse, you will provide individualized, patient-centered care while working closely with patients and their families:
- Deliver skilled nursing care in accordance with physician orders and care plans
- Assess, monitor, and document patient conditions, identifying physical and psychosocial changes
- Administer medications, treatments, and interventions, evaluating effectiveness
- Develop, implement, and adjust care plans based on patient needs (RN scope)
- Collect and interpret clinical data to support high-quality outcomes
- Communicate changes in patient status to physicians and care teams
- Educate patients and caregivers to promote independence, safety, and wellness
Qualifications
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts
- Current BLS/CPR certification
- Current TB test or chest X-ray
- Strong clinical skills, attention to detail, and a patient-first mindset
- One year of nursing experience preferred (new graduates encouraged to apply)
