Responsibilities
- Deliver skilled nursing care according to physician orders and care plans
- Administer medications, treatments, and interventions as prescribed
- Assess and monitor patient conditions
- Educate patients and family members on care and treatment plans
- Document care accurately and timely
- Collaborate with physicians, caregivers, and the healthcare team
- Report changes in patient condition promptly
Qualifications
- Current Pennsylvania LPN or RN license
- Current CPR certification (or willingness to obtain)
- Strong assessment, communication, and organizational skills
- Home health or pediatric experience preferred, but not required
- Compassionate and dependable with a patient-centered approach
- Reliable transportation
