What are the responsibilities and job description for the NEW LPN Visit Nurse Team - Homecare position at Maxim Healthcare Services?
Pay: $25 - $28 / per visit
Maxim Healthcare in Hebron is hiring for Licensed Practical Nurses (LPN) to work with patients in the homecare setting for a Visit Nurse Position, which includes quick 35 minute - 1 hour visits in the patient's home. Visits can include med pass, insulin assistance, wound care, etc. We are primarily looking for EVENING AND WEEKENDS
Intermittent Nursing Team Tier 1 – This nursing team will enjoy consistent weekly scheduling centered around an expected number of visits/days per week.
Expectations:
1. Be available 4 days per wk.
2. Be available E/O weekend or one weekend day per week
3. Complete a minimum of 32 visits per wk.
4. Compensation discussed on an individual basis.
Intermittent Nursing Team Tier 2 – This nursing team will enjoy consistent weekly scheduling centered around an expected number of visits/days per week.
Expectations:
1. Be available 3 days per wk.
2. Be available E/O weekend or one weekend day per wk.
3. Complete a minimum of 24 visits per wk.
4. Compensation discussed on an individual basis.
There is also a casual/ PRN option with no set availability expectations!

Responsibilities:
- Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
- Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
- Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
- Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
- Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
- Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable
Requirements:
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) for the state in which the nurse practices.
- Current Health Certificate (per facility RN contract or state regulation).
- Current PPD or Chest X-Ray.
- Current BLS card.
