LPN Homecare Nurse – Rockford, IL

Maxim Healthcare Services is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to provide one-on-one care for adult and pediatric patients in the comfort of their homes throughout the Greater Rockford, Illinois area.
Patients may require skilled nursing support, including G-tube, tracheostomy, and ventilator care. No homecare experience? We provide paid training and ongoing clinical support to help you succeed.
Full-time and part-time opportunities available.

Position Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care
- Develop and update individualized plans of care based on patient needs
- Deliver therapeutic, preventive, and rehabilitative nursing interventions
- Monitor patient conditions and communicate changes to physicians and supervisors
- Administer medications and prescribed treatments
- Document care and evaluate treatment effectiveness
- Educate patients and families on healthcare needs and treatment plans
- Collaborate with the care team to support care coordination, goal setting, and discharge planning
Qualifications
Required
- Current Illinois Licensed Practical Nurse (LPN) license
- Current BLS/CPR certification
- Current Health Certificate (as required by state regulations)
- Current PPD or Chest X-Ray
Preferred
- One year of LPN experience
