- Collaborate with RNs, physicians, and interdisciplinary care teams to develop and implement patient care plans
- Document patient assessments, treatments, and progress accurately in medical records
- Assist with patient admissions, discharges, and transfers in accordance with facility protocols
- Educate patients and families on care instructions, medications, and health management
- Respond promptly to patient needs and changes in condition, escalating concerns as appropriate
- Maintain a safe, clean, and compliant care environment following all regulatory standards
