Licensed Vocational Nurse (LVN) – Home Health (Full-Time)
Location: Uvalde, TX
Schedule: Full-time | On-call rotation required
We are currently seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Home Health team. In this role, you'll provide one-on-one patient care in the comfort of their homes, under the supervision of a Registered Nurse and according to the plan of care established by the physician.
Key Responsibilities:
- Perform baseline nursing care and clinical skills in accordance with agency policies and home health standards
- Maintain clinical competency and adhere to the state’s LVN practice act
- Identify patient needs, prioritize care, and anticipate potential complications or emergencies
- Administer medications and treatments safely and according to policy and best practices
- Communicate clearly with the RN, patients, families, and physicians
- Accurately document assessments, interventions, patient responses, and education in a timely manner
- Provide care with respect to patient rights, safety, dignity, and privacy
- Collaborate with the care team during case conferences and team meetings
- Assist with patient care coordination and maintain a positive, team-focused attitude
- Make scheduled visits and participate in on-call rotations as needed
Qualifications:
- Current LVN license in the state of Texas
- Home Health experience preferred but not required
- Strong clinical judgment and communication skills
- Reliable transportation and valid driver’s license
- Ability to work independently and manage time effectively
