JOB SUMMARY:
This role will work, autonomously and with collaboration, to improve the outcomes of individuals in Maury County who are suffering from Opioid Use Disorder. This will include collaboration and coordination with the healthcare system, the justice system, and outpatient addiction treatment programs. The navigator will focus on providing supportive services and resources to patients with suspected or confirmed substance use disorders and will provide individualized care, to include linkages to placements and services. This will include scheduling substance use-related appointments, referrals to treatment, and other important wraparound services. The role will function as a liaison between the physician, professional healthcare staff and other involved parties and will collaborate with the healthcare team to provide a seamless transition from hospital to home to additional treatment opportunities. Skills necessary for the role include excellent written and verbal communication skills, advocacy, problem solving in complex social situations, and the ability to work well with diverse individuals and teams. The availability and willingness to work occasional flexible hours and to expand outreach efforts into Maury and surrounding counties is necessary. The role will also be responsible for project management activities.
JOB REQUIREMENTS:
Associate's Related Field Required
3 Years behavioral health, mental health, and/or addiction treatment services Required
Experience and/or training in Opioid Use Disorder (OUD) Required
