Job Description
We are seeking compassionate, reliable, and detail-oriented caregivers to assist a client transitioning home from a skilled nursing facility. This role is vital in ensuring the client remains safe, comfortable, and supported during their recovery.
The ideal candidate will be comfortable following a structured care plan and maintaining accurate clinical logs.
Key Responsibilities
- Personal Care: Providing essential assistance with showering, toileting, and dressing.
- Mobility Support: Assisting the client in and out of bed and ensuring safety during transfers.
- Clinical Tracking: Recording daily vitals in a Weekly Clinical Log.
- Detailed Care: Following a specific Morning and Evening Daily Care Checklist to ensure all needs (nutritional, hygiene, and comfort) are met.
Requirements
- Proven experience in home care or a similar healthcare setting.
- Strong communication skills and an empathetic approach to senior care.
- Ability to maintain clear and accurate daily documentation.
- Reliable transportation to ensure punctuality for scheduled shifts.
- Georgia CNA.PCA, or HHA certification, preferred.
