Position Summary:
The Housing Case Manager will be responsible for providing outreach to homeless individuals at MTKO and case management of individuals and families served through the First HOPE Housing Program with a broad perspective of services including comprehensive case management and incorporating a Housing First philosophy, in a permanent supportive role. This employee must recognize the multiple and serious needs of persons who are homeless, the various subgroups of the homeless population, and the need for multiple interventions.
Essential Job Functions
- Provide outreach to MTKO clients, including but not limited to case management, crisis intervention, information, referral, accessing basic and emergency needs, advocacy, encouragement, and compassion.
- Provide regular home visits for program participants.
- Work with other professionals in the community in an advocacy role on behalf of MTKO clients and guests.
- Maintain professional boundaries in the provision of service, with a high level of emotional intelligence.
- Assist with outreach and referral as needed with priority given to case management.
- Respond appropriately to crises, and work in cooperation with the team to implement safety measures and procedures for reporting and ongoing documentation.
- Maintain confidential, updated, accurate statistics and individual client goal plans.
- Assist clients in the identification of strengths, needs, and goals, including meeting obligations of tenancy to maintain their home.
- Provide coaching and modeling, when necessary, on how to keep unit in good standing.
- Develop an individualized housing stability plan in cooperation with the client.
- Maintain current records in the Homeless Management Information System (HMIS) database and paper case files, documenting interactions.
- Assist clients in increasing financial security by accessing, maintaining, or improving employment and/or benefits.
- Assist program participants with critical skills related to household budgeting, money management, accessing a free personal credit report, and resolving personal credit issues.
- Provide problem-solving and crisis intervention services to empower clients to develop self-sufficiency.
- Provide case management services related to the housing needs of program participants in helping them obtain housing stability such as teaching/coaching clients to develop communication and self-advocacy skills.
- Assist in connecting households to appropriate resources and services and provide ongoing case management to ensure long-term housing stability.
- Utilize trauma-informed care and culturally competent principles in providing services to clients.
- Utilize Housing First and Motivational Interviewing models in service delivery.
- Determine program participant eligibility and assess annually including gathering all documents needed based on program guidelines.
- Assist program participants with accessing community resources including but not limited to transport to appointments.
- Monitor and evaluate program participant progress.
- Provide assistance with daily operations such as weekend on-call coverage.
- Help facilitate RentWise classes.
- Perform other duties as assigned.
- Adhere to all policies and procedures of MTKO.
Skills and Education
- Bachelor’s degree in human services, or related field, required.
- Three - five years of experience in the field of poverty, homelessness, domestic violence, mental health, and/or substance use, required. Personal life and work experience may be substituted for education.
- Proficient in Microsoft Office Suite.
- Genuine interest in and concern for all homeless and near homeless individuals and the ability to advocate for this population with other professionals in the community.
- Knowledge of street culture and comfort in engaging and building rapport with persons who are homeless.
- Ability to maintain a flexible work schedule and participate in a team environment and team-building activities.
- Excellent organizational skills.
- Excellent knowledge of community resources.
- Ability to communicate verbally and in writing effectively with diverse audiences.
- Valid driver’s license and proof of insurance are required as well as a good driving record which includes:
- No major violations including DUI, racing, hit and run, speeding more than 20 mph over the speed limit, or manslaughter.
- No more than 2 moving violations within the last 3 years.
- No more than 1 at-fault accident within the last 3 years.

Job Type: Full-time
Pay: $19.00 - $21.00 per hour
Schedule:
- Monday to Friday
- Weekends as needed
