Nurse Practitioner Role
We are seeking a skilled and compassionate Nurse Practitioner to join our team of clinicians who provide comprehensive health assessments to members across the nation. Responsibilities:
- Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Work with Primary Care Physicians (PCPs) on patient education and follow-up as necessary.
- Provide services at the member's place of residence, which can be in their homes or Skilled Nursing Facilities.

Qualifications:
- Master's degree or commensurate experience and satisfactory completion of NP licensure.
- Current NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority as needed.
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- One year of NP experience preferred, but new grads encouraged to apply.
- Valid state driver's license and proof of adequate automobile insurance coverage for the state of residence.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
Salary : $140,000 - $930,000
