We are seeking an experienced Registered Nurse to complete Start of Care assessments and skilled nursing visits throughout Titusville and the surrounding Brevard County area.
This is an excellent opportunity for a dependable RN who is comfortable working independently, managing patient care in the home setting, and coordinating with physicians, clinical staff, patients, and families.
Responsibilities
- Complete Start of Care assessments and initial patient evaluations
- Perform skilled nursing visits, reassessments, recertifications, and discharge visits
- Develop and update individualized plans of care
- Complete comprehensive nursing assessments
- Monitor patient conditions and report changes to the appropriate clinical team
- Provide wound care, medication management, patient education, and disease-management support
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals
- Complete accurate and timely documentation in the electronic medical record
- Follow OASIS requirements and home health documentation standards
- Educate patients and families regarding medications, treatments, and care plans
- Maintain compliance with agency policies, state regulations, and professional nursing standards
