Maternity Care Coalition (MCC) is hiring an HFA Case Manager IV (Advocate IV) to provide high-touch, in-person support to pregnant people and parenting families throughout our Pennsylvania service area—including, Bucks, Delaware, Lehigh, and Montgomery, and Philadelphia counties.
What You’ll Do:
- Provide intensive, weekly in-home visits to families navigating complex challenges like trauma, substance use, mental health concerns, and child welfare involvement.
- Empower families using evidence-informed parenting, health education, and child development strategies.
- Develop individualized Family Service Plans and connect families to vital resources—from health care to housing.
- Promote early childhood development and reproductive wellness through community outreach and parenting support.
- Document all interactions accurately and maintain records using MCC’s case management systems.

- Love being on the move. You’ll regularly travel across multiple counties and work face-to-face with families in their homes and communities.
- Embrace flexibility. This role requires non-traditional hours, including evenings and weekends, based on the based on the needs of the families served.
- Are a relationship-builder. You value trust, inclusion, and reflective supervision to support lasting impact.
Must-Haves:
- Associates, Bachelor's or Master's degree in Social Work, Public Health or related field.
- Valid driver’s license and clean driving record.
- Experience with case management, community health, or supporting families with complex needs.
- Strong documentation and organizational skills.
- Comfortable working independently
