Now Hiring: Home Health Patient Care Coordinator (PCC/Intake) | Home Health | Santa Clara/Gilroy/Peninsula Territory but the office is in San Jose-Must currently reside in the area as will need to travel through the territory as needed weekly.

This Home Health Patient Care Coordinator (PCC/Intake) role serves as the heart of communication, scheduling, and care coordination — ensuring patients receive timely, high-quality care while supporting the clinical team behind the scenes.

💼 Ideal for:

An organized, personable, and quick-learning individual with at least 1 years of experience in home health or other very similar healthcare industry (i.e. hospice, skilled nursing, hospital) experience in same type of role but looking to grow in the home health and elder care industry.

💻 Work Structure:

This position is mostly remote, with occasional time spent in the office in San Jose or out in the field within Santa Clara and Peninsula and Gilroy to support referrals, coordination, or team collaboration. As such you must reside in CA and be able to be on site if needed in an hour or so.

💰 Compensation:

$35–$38/hr quarterly bonuses career growth potential into salaried leadership roles
