Position Summary

They are seeking an experienced RN / Case Manager who thrives in the field and is ready to grow into a leadership role.

This role is ideal for a motivated nurse who wants to start with hands-on patient care and, over 1–2 years, transition to case management and team leadership responsibilities. You will work closely with the founder and small back-office team, supporting both patient care and operational oversight.

Key Responsibilities

• Provide high-quality, in-home patient care, primarily serving geriatric patients (mostly in their 80s).
• Manage your own case load with the opportunity to mentor and supervise staff over time.
• Collaborate with founder-RN and small administrative team (QA, scheduling, documentation).
• Support patient intake, client engagement, and sales as needed.
• Coordinate care with per diem staff, including RNs, OT, LVN, and home health aide.
• Document all visits and communications accurately using WellSky EHR.
• Serve a diverse patient population, including Mandarin-speaking clients.
• Handle occasional off-hours visits as needed.

Schedule & Environment

• Full-Time Preferred
• Core hours: 9:00 AM – 5:00 PM, with time for administrative duties
• Hybrid role combining fieldwork and office/admin responsibilities

Qualifications

• Active RN license in California
• Prior home health, case management, or field nursing experience preferred
• Demonstrated leadership potential with desire to grow into a management role
• Excellent communication, organizational, and clinical skills
• Comfortable working with diverse patient populations, including geriatric care
• Valid driver’s license, auto insurance, and reliable transportation
• Smartphone with GPS capability for visit documentation
• Fluency in Mandarin or Vietnamese a plus

Compensation & Benefits

• Base Salary: $140,000 – $160,000 DOE
