Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by observing vital signs and recognizing changes in condition
- Support medication management and administration following care plans and HIPAA regulations
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary
- Provide companionship to reduce feelings of loneliness and promote mental stimulation
- Maintain a clean and safe environment through light housekeeping, laundry, and organization
- Use specialized equipment such as Hoyer lifts or other assistive devices to ensure safe transfers and mobility
- Document patient observations, care provided, and any incidents accurately in compliance with privacy standards
- Collaborate with healthcare professionals to implement individualized care plans for clients with dementia, Alzheimer's, or developmental disabilities
- Support clients during community outings or social activities to foster engagement and independence
Qualifications
- Prior experience in home & community care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation, vital signs monitoring, medication administration, and dementia or memory care is essential
- Certification in CPR is required; additional certifications such as HIPAA training are a plus
- Experience working with the elderly, individuals with disabilities, autism spectrum disorder, or developmental disabilities is advantageous
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Strong communication skills to effectively interact with clients, families, and healthcare team members
- Demonstrated reliability, compassion, patience, and a proactive attitude toward caregiving responsibilities
