Key Duties: Communicate with residents and families about care plans and treatment protocols, participate in team meetings, monitor and adjust resident statuses, organize daily work, complete documentation, perform utilization reviews, coordinate admissions, discharges, and referrals, and maintain positive relationships with residents, families, staff, and providers.
Qualifications & Skills: Bachelor’s degree preferred; healthcare, case management, or social services experience strongly valued. Knowledge of Medicare, Medi-Cal, Medicaid, and electronic medical records is desirable. Strong communication, organization, time management, and computer skills required.
