We deliver endocrinology and diabetes care in an outpatient setting located on the Newton-Wellesley Hospital main campus (2014 Washington Street, in Newton, MA).
Our Endocrinologists Help People With Diabetes And Hormonal Disorders Successfully Manage Their Conditions. Patients Receive State-of-the-art Diabetes Treatment Through Our Team’s Advanced And Effective Techniques. Conditions Treated
Job Summary
Collaborating with physicians, nurses, medical assistants, and other healthcare professionals to ensure coordinated and patient-centered care, primarily handling phone triage, our nurses work in a collaborative and patient-centric environment, contributing to the delivery of high-quality healthcare services and are responsible for:
Patient Care
Working within a nursing pool to triage incoming phone calls and patient gateway messages and gather important clinical information.
Collaborating and communicating with providers to develop patient plan and communicating plans with patients by phone or EMR.
Connecting patient data to the EMR.
Prescription refills: Evaluating appropriateness of prescription refills and confirming current regimen. Evaluating need for prior authorization appropriateness.
Advocating for patient needs always
Staying informed on current standards of care in endocrinology and applying evidence-based practices in patient care.
Patient Education
Educating patients on endocrine conditions, diabetes self-management, lifestyle modifications, and the importance of adhering to prescribed treatment plans. This includes sick day rules for diabetes and adrenal insufficiency.
Educating patients on use of insulin pumps, continuous glucose monitoring systems, and glucometers.
Educating patients on self administering injectable medications
Providing clear and concise information on endocrine conditions, and ensuring patients have a thorough understanding of their health and treatment options.
