What are the responsibilities and job description for the Patient Navigator Complex Care Coordinator position at Mass General Brigham Community Physicians?
Qualifications:
Bachelor's Degree in related field of study required; relevant experience can be considered in lieu of degree.
1 years of patient facing, medical office or other related experience strongly preferred.
Bilingual Spanish speaking strongly preferred.
Additional Skills for Success:
Strong knowledge of healthcare resources, community services, and patient advocacy.
Excellent communication and interpersonal skills.
Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.
Exceptional organizational and time management skills.
Familiarity with electronic health records and case management software.
Schedule and Work Model
Full-time, Monday through Friday, approximately 8am-4:30pm.
Hybrid work model: Flexibility to work on-site at Chelsea Health Center up to 4 days per week.
Remote workdays require stable, secure, quiet, compliant workspace with MGB issued equipment.
