Temporary | Days | Monday - Friday from 8am - 4pm
Masonic Villages of Pennsylvania - Home Health, Hospice, and Home Care Campus - is immediately hiring a Social Worker for Hospice.
What You Need as a Social Worker for Hospice:
- Graduation from a four year college or university with a degree in social work. BSW required.
- MSW preferred
- Personal computer experience required
- Hospice or Medical Social Work experience preferred
- Maintains a current PA driver' s license and insurance on personal vehicle
What You'll Do as a Social Worker for Hospice:
As a Social Worker for Hospice, you will carry out our Mission of Love by assisting patients, families and caregivers with emotional and practical issues at the end of life with the goal of enhancing quality of life.
Our Social Worker for Hospice is responsible for the following:
- Assures that social service activities are integrated and coordinated within the hospice.
- Develops specific, measurable, realistic plans and objectives with the goal of quality of life and safety in mind.
- Performs psychosocial and bereavement assessments on all patients and families.
- Serves as a resource to patients and families for questions and emotional support. Identifies and addresses psychosocial needs of patients and families.
- Demonstrates knowledge of clinical social work.
- Communicates and collaborates appropriately and clearly to all members of the hospice and facility/referral source teams.
- Facilitates admission meetings and demonstrates competency and proficiency in explaining Medicare hospice regulations, advanced care planning, and hospice education for patients, families, and referral sources.
- Demonstrates the ability to be flexible, organized and function under stressful situations.
- Makes referrals to outside agencies or groups as needed.
- Demonstrates cultural competency in work.
- Documents in accordance with Medicare regulations and hospice department standards.
- Is an active member of team meetings.
- Participates in committees and projects as appropriate.
- Carry out QAPI roles and responsibilities as assigned.
- Assist in overseeing volunteer program.
