Primary Care Physician
We are seeking a dedicated, patient-focused Primary Care Physician to
join a growing, value-based care practice serving a predominantly geriatric
population. This is a full-time, 100% outpatient opportunity offering a
collaborative, team-based environment with a strong emphasis on preventive
care, chronic disease management, and improving quality outcomes for seniors.
This role is ideal for a physician who is passionate about delivering
high-quality, relationship-driven care and is comfortable working with Medicare
Advantage patients in a model that prioritizes access, coordination, and
long-term health outcomes.
Position Highlights
- Monday–Friday schedule, 8:00 AM –
5:00 PM (no weekends)
- 100% outpatient setting

patients)

Key Responsibilities
- Provide comprehensive primary
care to a geriatric patient population with multiple chronic conditions
- Deliver patient-centered care
with an emphasis on preventive services and care coordination
- Develop and manage individualized
care plans in collaboration with patients, families, and interdisciplinary
teams
- Utilize virtual care tools to
monitor and manage high-risk patients between visits
- Ensure accurate and compliant
documentation, including risk adjustment and quality metrics coding
- Communicate effectively with
patients, caregivers, and other providers across the continuum of care
- Support improved clinical
outcomes and patient satisfaction through proactive engagement
Qualifications
- MD or DO from an accredited
medical school (Family Medicine, Internal Medicine, or Geriatrics
preferred)
- Board Certified or Board Eligible
strongly preferred
- Active and unrestricted Florida
Medical License
- DEA license
- Current CPR certification (BLS
required; ACLS/PALS preferred)
- Experience managing geriatric
patients and Medicare Advantage populations required
- Familiarity with value-based care
models, risk adjustment (MRA), and quality measures strongly preferred
