Job Summary:
We are looking for an adaptable Post Acute Primary Care Physician (MD/DO) to oversee our post-acute clinical services across two key locations: Melbourne and Merritt Island, FL. This position focuses on delivering exceptional, hands-on care through daily rounds at our partner Skilled Nursing Facilities (SNFs). You will serve as the primary clinical leader on-site, managing higher-risk patients and supervising a dedicated team of Nurse Practitioners and Physician Assistants.
Schedule:
Hours: Monday to Friday, 8:00 AM – 5:00 PM
Key Responsibilities:
Conduct daily rounds splitting coverage between two facility locations in Melbourne and Merritt Island.
Perform initial assessments and evaluations for all newly admitted patients.
Manage acute updates and coordinate care plans for higher-risk, fragile patients.
Monitor, supervise, and collaborate closely with regional Advanced Practice Providers (APPs).
Qualifications & Requirements:
Board Certified (BC) or Board Eligible in Family Medicine or Internal Medicine (Board Certification is NOT a requirement).
Active Florida Medical License.
ACN (Area of Critical Need) license holders are accepted and welcome to apply.
Ability to commute seamlessly between facilities in Melbourne and Merritt Island.
Compensation & Benefits:
Base Salary: $225,000 – $245,000 per year
Time Off: 2 weeks (14 days) of Paid Time Off (PTO)
