We are seeking an individual with experience in the medical field that has a strong desire to deliver high quality, exceptional care. Excellent communication, organization, and customer service skills are required. The LVN partners with the interdisciplinary care team to provide care coordination for complex patients. This role requires an exceptional level of critical thinking and organization. We specialize in engaging and caring for the top 1% highest utilizing Medicaid patients. We are looking for empathetic professionals that are enthusiastic about this population with a focus of care from that person's perspective.
Compensation and Benefits for Licensed Vocational Nurse
Compensation: $65,000 - $85,000 annually
3 weeks (2 weeks 6-7 federal holidays) paid vacation and 401K

Responsibilities for Licensed Vocational Nurse
Employs a person-centered care approach to perform the assessment, treatment planning and ongoing care management of eligible participants.
Makes timely referrals and facilitates referrals for indicated services (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, medical referrals, substance abuse treatment etc.).
Utilizes motivational interviewing to engage and educate participants, and coordinate follow up care.
Qualifications for Licensed Vocational Nurse
2 Years or more experience as a Licensed Vocational Nurse
Unencumbered Licensed Vocational Nurse License
Bilingual (preferred)
Salary : $65,000 - $85,000
