Job Overview

We are seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our multidisciplinary healthcare team. Duties

  • Conduct thorough patient assessments to identify psychosocial needs, including mental health conditions, behavioral health issues, and family dynamics.
  • Provide individual, group, and family counseling utilizing evidence-based approaches such as cognitive behavioral therapy, motivational interviewing, and behavioral therapy techniques.
  • Coordinate with healthcare providers to develop and implement discharge plans that address social determinants of health and facilitate smooth transitions of care.
  • Manage case documentation accurately within electronic medical record (EMR) systems while ensuring compliance with HIPAA and clinical ethical guidelines.
  • Collaborate with community resources to connect patients with appropriate services such as hospice care, assisted living facilities, or developmental disability programs.
  • Participate in clinical research initiatives related to social work practice standards and healthcare outcomes.
  • Support patients experiencing complex challenges such as addiction, dementia, PTSD, or end-of-life care through specialized counseling and intervention strategies.
  • Engage in crisis intervention when necessary to ensure patient safety and stabilize acute mental health episodes.
  • Assist with intake procedures for new patients by conducting diagnostic evaluations and gathering relevant medical documentation.

Skills

  • Extensive knowledge of social work practice standards, medical terminology, ICD coding (ICD-9/10), and CPT coding for billing purposes.
  • Proficiency in behavioral health techniques including cognitive behavioral therapy, applied behavior analysis, and trauma-informed care.
  • Experience working with diverse populations such as children, geriatrics, individuals with disabilities or developmental disorders, and those requiring hospice or palliative medicine services.
  • Strong skills in community resource coordination and discharge planning to optimize patient outcomes post-treatment.
  • Familiarity with EMR systems for data collection, documentation, and medical records management.
  • Ability to perform comprehensive patient assessments while adhering to HIPAA regulations and clinical ethical guidelines.
  • Excellent communication skills for effective collaboration with multidisciplinary teams including nursing staff, physicians, therapists, and social service agencies.
  • Demonstrated crisis management capabilities along with experience working in inpatient settings, nursing homes, outpatient clinics, or home & community care environments.
  • Knowledge of Medicare policies and experience working within hospice & palliative medicine frameworks is highly desirable.
