- Prepares medication orders by admixing medications under the supervision of a physician, calculating correct dosage volumes
- Checks all orders for completeness of information, verifies patient information and proper physician authorization
- Checks all orders for insurance approval before admixing
- Properly disposes all materials used when mixing chemo or non-chemo IVs. Cleans hood daily or as stated in policy and procedures
- Logs and dates all drugs being used. Checks drugs for monthly outdate and other quality assurance activities as assigned.
- Maintains proper control documentation of medications. Completes inventory control of medications
- Performs efficient data entry into systems used by admixture.
- Orders medication and supplies as directed
- High school diploma or equivalent required
- Three to five years experience in a pharmacy with IV skills preferred
- IV certification strongly preferred
- Experience with Lynx, Epic, Pyxis, Omnicell is preferred
- Certified Pharmacy Technician certificate or State Pharmacy Board preferred
- Willing to travel between offices as needed
