In partnership with the Maryland Department of Health, Behavioral Health Administration, the 211 Care Coordination program facilitates referrals for hospital patients in need of community-based behavioral health services, including resources for mental health conditions, substance use disorders, and developmental disabilities.

The Care Coordinator, Inpatient Systems supports inpatient providers by helping coordinate connections to appropriate behavioral health resources and strengthening referral workflows within the 211 Care Coordination program. This role works closely with hospital teams and care coordination staff to promote timely discharge planning, reduce barriers to community placement, and ensure patients are linked to the services they need after hospitalization.

Collaborating with the Care Coordination Program Manager and operational care coordination staff, this position supports efforts to improve system navigation, streamline processes, and strengthen communication between inpatient and community-based providers. While the role does not include supervisory responsibilities, it plays an important part in enhancing day-to-day coordination and supporting ongoing workflow improvement.

The ideal candidate brings strong knowledge of behavioral health services, a collaborative approach to problem-solving, and the ability to support inpatient teams in delivering effective, patient-centered transitions of care.

EDUCATION AND EXPERIENCE

  • Bachelor’s Degree with at least 5 years of experience in social work, public health, healthcare, or human services required; Master’s Degree preferred.
  • Minimum of 5 years of experience working within behavioral health systems, care coordination, or clinical program development.
  • Advanced knowledge of Maryland’s Public Behavioral Health System strongly preferred.
  • Experience working with Maryland Department of Health (MDH), Department of Human Services (DHS), Department of Social Services (DSS), Developmental Disabilities Administration (DDA), or similar systems preferred.
  • Experience providing training, consultation, or workflow development strongly preferred.

POSITION RESPONSIBILITIES INCLUDE:

Care Coordination

  • Support coverage of the 211 Maryland Hospital Care Coordination referral line Monday – Sunday, 8am – 8pm, based on schedule.
  • Receive and respond to incoming phone and electronic behavioral health patient referrals from Maryland hospitals for inpatient placements and outpatient community-based services within defined timeframes established in grant deliverables.
  • Collaborate with hospital staff during defined intervals to conduct bed searches, review referral statuses, follow up on denials, and coordinate communication between 211P4 and hospital teams to reduce staff burden.
  • Coordinate with involved agencies (e.g., DDA, LCT teams) upon hospital notification of a boarder to support timely resource engagement and care transitions.
  • Complete and submit necessary documentation (e.g., outpatient referrals) on behalf of hospitals to minimize administrative workload.
  • Utilize the 211 Resource Database to identify available services, connect with providers, and facilitate warm handoffs.
  • Connect families to peer-to-peer support and initiate community-based services while the patient is still in the ED or inpatient unit to promote smooth discharge and community reintegration.
  • Conduct follow-up within 24–48 hours of patient placement to ensure successful service connection and identify additional coordination needs.
  • Regularly communicate placement progress and updates with referring hospitals.
  • Thoroughly document care coordination activities in Maryland Information Network’s designated technology systems.
  • Securely maintain confidential patient records, assessments, and related documents.
  • Attend program-related meetings and required training.

Systems Consultation & Development

  • Provide subject matter insight related to behavioral health systems, service navigation, and complex care coordination processes.
  • Develop, refine, and support implementation of behavioral health care coordination workflows aligned with program goals.
  • Provide consultation and strategic guidance on high-acuity or systemically complex behavioral health cases.
  • Identify system gaps, operational barriers, and service limitations; recommend process improvements to enhance patient transitions and access to care.
  • Develop protocols, tools, and internal guidance materials to support consistent care coordination practices.
  • Support onboarding and skill development related to specialized workflows and complex case strategies.

TECHNOLOGY:

  • Software includes Microsoft Office Suite (required), Google Suite (required), and Atlassian (required).

ADDITIONAL INFORMATION:

This is the primary remote position. There may be times for in-person meetings, in-person training or other events where attendance will be required. Occasional walking or standing and interaction with personnel located in other parts of the building are also necessary when in the office.
