Description

Position Summary

The employee in this classification, under the direct supervision of the Director of Nursing or assigned supervisor, coordinates the development and completion of the resident assessments, in accordance with current Federal, State, and local standards, guidelines and regulations that govern the Facility, as well as corporate compliance requirements and established Facility policies and procedures; performs related duties as assigned.

Essential Functions

  • Conduct and coordinates the development and completion of resident assessments Minimum Data Set (MDS), in accordance with current Federal, State, and local standards, guidelines and regulations that govern the Facility, as well as corporate compliance requirements and established Facility policies and procedures. Consults with the resident’s physicians, consultants, and governmental agencies that may be involved in providing the resident assessment/care plan functions of the facility.
  • Coordinates and/or completes resident assessments on admission, quarterly, annually, or upon change of condition, Resident Assessment Profiles (RAPS), Triggers; initiates and updates care plans.
  • Utilize MxManage Program (or equivalent) and follow up on discrepancies/opportunities.
  • Assists Director of Nursing with prompt input of Level of Care Assessment (LOC) into the state system to ensure that residents qualify for nursing home care.
  • Assists Director of Nursing with transmission of assessments to the State on a weekly basis, or as indicated to ensure facility compliance. Perform computer/data processing including entering data and transmitting to the State and correction of MDS errors according to the validation report.
  • Assists Director of Nursing with reviews of Daily Census to ensure needed (MDS) assessment, or tracking forms (i.e. re-entry/discharge) are inputted into the computer. Informs all Interdisciplinary Team members of the requirements and completion of the resident assessment (MDS), to ensure completion and transmission of document. Coordinates, complies, and distributes (MDS) weekly due list for the Interdisciplinary Team members. Assists Director of Nursing with staff scheduling of the resident/care plan meeting, (RCC) and reviews tickler file after every transmission, to ensure compliance.
  • Assists Director of Nursing with review all potential Medicare and HMO admissions to determine the level of care and potential reimbursement. Assists Director of Nursing in determining and communicating MDS assessment reference date to maximize reimbursement. Assists Director of Nursing with maintaining Medicare calendar and monitoring Medicare residents to assess care level, and possible use of grace days to maximize reimbursement.
  • Act as Co-Chairperson of the Interdisciplinary Care Plan Team as indicated. Conducts or coordinates the interviewing of each resident and/or family for the resident’s assessment to develop a comprehensive resident assessment and care plan for each resident.
  • Collaborates with the Interdisciplinary Team on residents who are re-admitted for a change of condition or upon significant change; initiates the MDS as indicated.
  • Participate in functions involving discharge plans as may be necessary.
  • Serve on, participate in, and attend various other committees of the facility (e.g., Quality Assessment and Assurance) as required, and as appointed by the Director of Nursing. Assists Director of Nursing with developing, implementing, and maintaining the Facility’s on-going quality assurance program for resident assessment/care plans. Evaluate and implement recommendations from established committees as they may pertain to the assessment and/or care plan process of the facility. Provide written and/or oral reports of the resident assessment/care plan functions as requested or directed by Director of Nursing.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Assist the Director of Nursing and Facility Educator in developing any training activities needed concerning resident assessment/care plan skills (including, but not limited to initial or refresher courses relative to techniques for interviewing residents, rehabilitation principles, commonly used psychotropic drugs, care plan functions, etc).
  • Assists Director of Nursing in the development and revisions to the Nursing Department Policy Procedure Manual based on current literature and/or best practice.
  • Provides leadership to Facility nursing staff; is responsible for observing the practice of other licensed or unlicensed nursing staff. Observes staff providing direct care to residents to ensure compliance with the resident’s care plan and wishes, reports and documents concerns.
  • Assists the Director of Nursing in reviewing resident charts and identifying discrepancies noted concerning physician’s orders, diet changes, charting error, for appropriate follow through; makes periodic checks to ensure that prescribed treatments are being properly administered and to evaluate the resident’s physical and emotional status.
  • Participates in employee performance evaluation by providing input and making recommendations to the Unit Manager or Director of Nursing concerning nursing staff. Participates and initiates documentation for Code of Conduct violations.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, audits, charting, etc, as necessary.
  • Bends, stoops, reaches and stands long periods of time while providing direct nursing services.
  • Lifts, moves and carries equipment and supplies that weigh up to 50 pounds; pushes and pulls equipment and supplies that weigh up to 100 pounds.
  • Manual dexterity for administering injections and other nursing procedures.
  • May require reassignment by the Facility to provide direct nursing services in order to maintain appropriate levels of professional staff.

Qualifications


Education, Training and Experience

  • A Bachelor Degree in Nursing from an accredited college or university is preferred.
  • Be licensed by the Michigan Board of Nursing to practice as a Registered Nurse, and must maintain licensure.
  • A minimum of two years of previous (MDS) experience and (MDS) certification is (preferred).
  • Be physically able to perform the essential functions of the position, with or without reasonable accommodation.

Additional qualifications

  • Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures that are necessary.
  • Knowledge of nursing principles and methods; familiarity with organization, functions, policies, regulations and procedures long-term medical care facilities.
  • Ability to use good professional judgment for the welfare and safety of residents and employees; acts as a resident advocate.
  • Knowledge in Quality Indicator Data Profiles.
  • Knowledge of skilled Medicare regulations related to reimbursement.
  • Knowledge of applicable computer applications.
  • Must be able to read, write, speak, and understand the English language.Ability to make independent decisions when circumstances warrant action.
  • Ability to establish and maintain effective working relationship with residents, residents’ families, Facility staff, health professionals, physicians, consultants, and governmental agencies.
  • Ability to maintain the confidentiality of the resident care.
  • Ability to conduct oneself with tact and courtesy.

The qualifications listed above are guidelines. Other combinations of education and experience which could provide the necessary knowledge, skills and abilities to perform the job may be considered.

Physical Requirements

Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
