Reports to and works closely with the Coordinator of Case-Management and Student Development and other administrative staff.Works closely with the Director, Assistant Director, and Coordinators to ensure quality care is being provided to clients.Participates in the on-call schedule and managing crisis situations.Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.Counseling Center outreach eventsTracking events and goal progress for the Counseling CenterScheduling meetings and sending out Teams links weeklyWorks closely with the Mental Health Specialists:Carry caseload for med management and higher acuity clients for more hands on servicesWalk-in clinic and intakesAssessmentDeveloping, monitoring, and evaluating treatment plans and progressCommunicate client needs to a variety of care team members and follow up accordingly and provide appropriate referralsLinkage and referring students to community resourcesMaintain client records by reviewing case notes and monitoring filesAdvocate for client’s successScheduling/rescheduling psychiatric appointmentsPsychiatric medication refillsWeekly psychiatric provider reports and weekly communication with medical staffMonitor psychiatric appointments and communicate any concerns with Mental Health Specialists and medical staffDocument and monitor student attendance, outreach to clients who are missing appointments to ensure continuity of careManage discharge plans upon completion of treatmentAssist in development and carrying out workshops and support groups
