Registered Nurse – Hospice Case Manager
Location: Sheboygan, WI (Field-Based / Home Hospice)
Start: 01/26/2026
Pay range : $70.00/hr - $80.00/hr
Schedule: Monday–Friday | 8:00 AM – 4:30 PM
Hours: 40 hours/week
Role Overview:
Seeking an experienced Hospice RN Case Manager to manage a home hospice caseload across Sheboygan County, with occasional support in Manitowoc County. This role includes care coordination, admissions, and end-of-life planning for adult hospice patients.
Key Responsibilities:
- Manage a hospice caseload of 13–15 patients
- Provide end-of-life care in homes, ALFs, and SNFs
- Perform hospice admissions and ongoing care coordination
- Pain and symptom management, wound assessments, triage
- Accurate documentation using Epic Remote Client
Requirements:
- Active WI or Compact RN license
- Minimum 2.5 years hospice case management experience
- Epic Remote Client experience required
- Prior travel experience preferred
- BLS certification
- Valid driver’s license and insurance
Patient Population:
Adult hospice / end-of-life patients
Additional Details:
- Weekend rotation: 1 weekend per month
- Call: 1 call shift per month
- No holiday requirements
- Floating within Sheboygan / Manitowoc counties as needed
- Orientation provided (up to 1 week)
Salary : $70 - $80
