Key Responsibilities
Leadership & Team Management
- Directly supervise and mentor the Coordination Department, including Coordinators, Scheduling Coordinators, and related administrative staff.
- Recruit, onboard, train, and evaluate coordination personnel.
- Establish departmental performance expectations and accountability measures.
- Conduct regular team meetings, coaching sessions, and performance reviews.
- Foster a positive, solution-oriented work environment.
- Oversee daily scheduling operations for all clients and caregivers.
- Ensure clients receive uninterrupted services with minimal missed visits.
- Manage high-volume scheduling while responding to last-minute call-outs and emergencies.
- Review schedules to maximize efficiency while minimizing overtime and travel.
- Ensure continuity of care by matching caregivers appropriately based on client needs, skills, language, and preferences.
- Monitor service authorizations and ensure scheduled hours remain compliant.
- Develop and improve coordination workflows and standard operating procedures.
- Monitor key operational metrics including:
- Fill rates
- Missed visits
- Open shifts
- Overtime
- Caregiver utilization
- Client satisfaction
- Identify operational bottlenecks and implement process improvements.
- Collaborate with HR, Nursing, Intake, Payroll, and Compliance departments to maintain seamless operations.
- Ensure compliance with all New York State Department of Health regulations and agency policies.
- Verify caregivers maintain current credentials before scheduling.
- Maintain accurate scheduling and documentation within the agency software system.
- Participate in audits and quality assurance reviews.
- Address scheduling concerns and client service issues promptly.
- Assist with conflict resolution involving caregivers or clients.
- Maintain professional relationships with referral sources and community partners when appropriate.
- Promote exceptional customer service throughout the coordination team.
- Partner with executive leadership to develop operational goals.
- Assist with agency growth initiatives and expansion into new service areas.
- Analyze scheduling trends and workforce utilization to improve operational efficiency.
- Recommend staffing strategies to support census growth.
Required
- Bachelor's degree in Healthcare Administration, Business Administration, Human Services, or a related field (or equivalent experience).
- Minimum of 5 years of home care experience.
- Minimum of 3 years in a supervisory or management role.
- Strong knowledge of New York State home care regulations.
- Experience managing high-volume scheduling operations.
- Excellent organizational and time management skills.
- Strong leadership and employee development abilities.
- Excellent communication and conflict resolution skills.
- Proficiency with Microsoft Office Suite.
- Ability to work in a fast-paced environment while managing multiple priorities.
- Experience with HHA Exchange, Sandata, AlayaCare, CareSmartz360, AxisCare, or similar home care software.
- Bilingual (English/Spanish, Haitian Creole, or Yiddish) is a plus.
