We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Maris Grove by Erickson Senior Living. As an LPN, you will play a vital role in providing high-quality clinical care to our residents, focusing on their physical, spiritual, emotional, and clinical needs.
Key Responsibilities:
- Developing relationships with residents and their families, utilizing a consistent caregiver approach.
- Assessing, observing, and evaluating residents' clinical conditions, documenting findings in the electronic medical record, and notifying providers/RNs as necessary.
- Updating or modifying care plans to reflect changes in residents' care and service needs.
- Supervising and leading care associates.
- Participating in meetings related to resident outcomes and care.
- Responding to residents, family members, or responsible parties in a timely and professional manner.
- Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
Requirements:
- Graduate of an approved Licensed Practical/Vocational Nursing program.
- A minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
- A passion for caring for seniors and being a strong resident advocate.
- Experience with electronic medical records is preferred.
