Job Description Summary

The emergency medicine pharmacist actively participates in patient care decisions, drug therapy consultations, and collaboration with the emergency department health care team. She/he prospectively evaluates all drug therapy for appropriate indications, dosage, drug interactions, contraindications and drug allergies; monitors the patient’s pharmacotherapeutic regimen for effectiveness and ADEs; and intervene as needed. She/he will serve as the drug information expert to the emergency department. She/he contributes to therapeutic decision-making by participating in patient care, particularly pharmaceutical care, providing drug information, formal and informal presentations and consultations on medication use in the hospital. She/he assures the accurate and efficient distribution of drugs and oversees the usage of pharmaceuticals within the facility. She/he also promotes rational, cost-effective use of drugs within the facility while assuring the quality of pharmaceutical care and optimal clinical outcomes. In addition, the emergency medicine pharmacist provides authoritative drug information to other health care professionals, patients and the public.

Job Specifications

Job Requirements, Prerequisites and Essential Functions:

Education

  • Requires level of education necessary for licensure as a pharmacist by the California Board of Pharmacy.
  • Prefer graduation from an accredited school of Pharmacy.
  • Pharm.D. degree required.
  • Completion of a Post Graduate Year 1 (PGY1) general practice residency and or Post Graduate Year 2 (PGY2) emergency medicine or critical care residency, or Post Graduate Year 1 (PGY1) general practice residency and a minimum of 3 years inpatient pharmacy staffing experience, required

Experience

  • Two years experience in an acute care hospital pharmacy including residency preferred.
  • 1-3 years in Emergency Medicine required.

License And Certifications

  • Licensure as a pharmacist in the State of California in good standing required
  • BCPS, preferred
  • BLS, required
  • ACLS, required
  • PALS, required

Prerequisite Skills

  • Current knowledge of drugs, their composition, actions, uses, toxic effect, dosage, availability and distribution. Ability to prepare sterile solutions. Familiarity with Federal and State laws regulating the profession of pharmacy and handling of drugs. Superior ability to work tactfully with patients, public, physicians, and other members of the healthcare team, and skillfully supervise daily workflow and personnel. Ability to demonstrate initiative, and problem identification and solving skills. Excellent written and verbal skills for teaching, presentations and persuasion. Demonstrated organizational skills. Knowledge of pathophysiology and disease states required to evaluate complex therapeutic issues and to monitor and advise on drug therapy.
  • Knowledge and understanding of Joint Commission standards and California board of pharmacy rules and regulations, required.
  • Ability to assist with cost saving initiatives and patient care optimization with respect to medications.

Duties And Responsibilities

Essential (Not Modifiable)

Major Position Responsibilities

  • Provides cost-effective, evidence-based pharmaceutical care to emergency room patients.
  • Discuss medication order changes/clarifications with the internist or the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes.
  • Provide pharmacokinetic consultations and clinical consultations as required.
  • Review, monitor, document and report adverse drug reaction and medication errors. Assists in their management and prevention, developing process improvements to reduce drug errors and preventable ADEs.
  • Provide drug information and intravenous compatibility information to the emergency team and uses the regional poison information center when indicated.
  • Oversee medication intake coordinators in the emergency department to ensure medication histories are accurate and complete.
  • Promote the use of the formulary, when possible, but coordinates procurement of non-formulary drugs when necessary.
  • Provide drug therapy-related education to emergency team member and pharmacy staff.
  • Documents clinical activities that include, but are not limited to, disease-state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education, and other patient care activities.
  • Acts as a liaison between pharmacy, nursing, and the medical staff to educate health professionals regarding current drug-related procedures, policies, guidelines, and pathways.
  • Provides consultation to hospital committees, when emergency medicine pharmacotherapy issues are discussed
  • Works closely with Clinical Manager and participates in quality assurance programs and clinical pathway development to enhance pharmaceutical care.
  • Provide operational support to the pharmacy when needed.
  • Develop and conduct target drug programs and drug usage evaluations as needed and report results to the Pharmacy and Therapeutics Committee.
  • Maintain BLS, ACLS/PALS certification and respond to all resuscitation events during scheduled shift, including code strokes.
  • Maintain knowledge of current primary and tertiary references pertinent to emergency medicine\
  • Lead and participate in process improvement (PI) projects
  • Review automated dispensing machine overrides and inventory (optimization)
  • Develop QA programs to ensure quality patient care and compliance with clinical/operational programs
  • Participate in development of and approval process of standard order sets
  • Test and validate build associated with Cerner requests and upgrades
  • Provide staff development
  • Educate other disciplines: Nursing orientation. Patient Safety orientation. Prescribers and House Staff, Other
  • Coordinate and participate in medication reconciliation and patient education
  • Review medication errors for area of responsibility and develop and implement process improvement plans as needed
  • Coordinate and participate in Medication Management Rounds
  • Develop and report metrics for work in area: Align metrics to departmental, service line and organization goals, Develop a format and report that can be reviewed by leadership and presented to staff
  • Complies with applicable laws, regulations and standards of practice.
  • Promotes a positive, supportive environment within the department and hospital by demonstrating a pleasant and helpful attitude.
  • Oversees and participates in drug distribution process in inpatient, outpatient and IV additive areas of the pharmacy.
  • Attends staff meetings, contributing to meeting agendas, discussions and problem resolution.
  • Contributes, via presentation, to the Pharmacy & Therapeutics Committee.
  • Directs workflow, including technical, billing and distributive functions, particularly in the absence of the immediate supervisors designated for these functions.
  • Executes responsibilities within the established guidelines of hospital and departmental policy & procedures.
  • Participates in department projects or committees.
  • Participates in hospital committees as assigned.
  • Ensures collection and assembly of departmental quality assurance data, as well as instruction within the department to improve or ensure threshold compliance.
  • Participates in the development and execution of drug utilization reviews.
  • Participates in the training and orientation of new employees as necessary.
  • Identifies needs and provides education to pharmacy staff and other departments via informal and formal methods, such as in services and newsletters.
  • Responds to Codes and Group Alerts with appropriate participation.
  • Regular attendance at interdisciplinary patient care meetings, and other applicable meetings, committees, etc.

Safety and Infection Control Responsibilities

  • Responsible to maintain a safe and clean work environment, including unit based specific safety and infection control requirements.

Secondary (Modifiable)

  • Executing routine monthly floor stock inspections for nursing units.

Compensation

The posted pay range complies with applicable law and reflects what we reasonably expect to pay for this role. Individual pay is set by skills, experience, qualifications, and internal/market equity, consistent with MarinHealth’s compensation philosophy. Positions covered by collective bargaining agreements are governed by those agreements.
