JOB SUMMARY:
The Diabetes Nurse Specialist will work in both the outpatient and inpatient setting, acting as a member of the interdisciplinary team that facilitates the continuum of care and education of patients with diabetes. Assess diabetic patient and pertinent family members’ knowledge of diabetes and develops and implements a patient-centered care plan; monitors and evaluates, as needed, resource utilization, patient outcomes, patients’ skills in self-care, and ability/readiness to learn. The position communicates with providers regarding patient needs and ongoing status of care plan. The Diabetes Nurse Specialist provides clinical resource and consultation to staff and demonstrates advanced clinical competence in diabetic care.

EDUCATION:

  • Active RN License, BSN preferred, BS required.
  • Certified Diabetes Educator (CDE) required or actively working towards CDE certification within 1000 hours and 2 years. If no CDE, needs 6 hours diabetes specific CEU a year to achieve their CDE.

EXPERIENCE:

  • Inpatient and outpatient nursing experience, with clinical expertise in diabetes care.
  • Clinical expertise in diabetes care with insulin pump/CGMS experience preferred.
  • Working knowledge of American Diabetes Association (ADA) recognition for Diabetes Self-Management Education (DSME).
  • Expert knowledge of key patient care standards and compliance specifications, Title 22, and Joint Commission.
  • Experience in electronic medical records, data management and analysis preferred
  • Actively working with a CDE community mentor (if non CDE individual).

LICENSE/CERTIFICATION:

  • Current California RN license, with diabetes clinical focus/CDE or actively working toward CDE certification.
  • BLS-HCP within 90 days of hire.

RESPONSIBILITIES:

  • Provides diabetes self-management education to both inpatients and outpatient.
  • Facilitates referrals to interdisciplinary care team, including community clinic, dietitian and endocrinologist. Assist in staff education including new nurse orientation, grand rounds, and CME events.
  • Assesses all transfer employees to assure competency of new staff.
  • Participates in hospital-wide RN, LVN, and Tech training program(s).
  • Assists in the development of instructional material for use in diabetes educational programs (includes objectives, content, editing, design, printing and distribution).
  • Demonstrates clinical expertise in diabetes management by serving as a clinical expert, role model and clinical consultant to patient care staff, physicians and other ancillary staff as appropriate.
  • Participates with other members of the Diabetes Care team in the preparation and updates of manuals, guides and other materials contributing to improving clinical performance in diabetes management, updating staff instruction and the maintenance of national, hospital and departmental
  • Other duties as assigned
