Job Description Summary

The Lead Diabetes Nurse Specialist provides lead support for the Inpatient Diabetes Care Program and the Advanced Inpatient Disease Specific Joint Commission Certification. The Lead Diabetes Nurse Specialist ensures the care of the patient with diabetes (PWD) throughout the continuum by working with the interdisciplinary team members to ensure comprehensive care of the PWD. The Lead Diabetes Nurse Specialist works seamlessly with the Diabetes Coordinator and Diabetes Care Program Manager to ensure all aspects of the Advanced Inpatient Disease Specific Joint Commission Certification are met, including but not limited to data collection, abstraction, validation, clinical interpretation and reporting.

Job Requirements, Prerequisites And Essential Functions

EDUCATION

  • Graduate from an accredited school of nursing with current licensure in the State of California.
  • Bachelor of Science in Nursing (BSN) degree or Master of Science in Nursing (MSN) degree preferred.

Experience

  • Experience with Joint Commission Inpatient advanced diabetes care is required
  • Significant clinical experience in the field of inpatient diabetes care is required including the ability to interpret data from the clinical record
  • Experience in electronic medical records, data management and clinical analysis is required
  • Experience with individual and group teaching methodologies is required

C. PREREQUISITE SKILL

  • Knowledge of clinical care and/or guidelines for endocrine diagnoses including but not limited to diabetes (all types) and stress hyperglycemia
  • Demonstrated teaching and leadership abilities
  • Demonstrated oral and written skills
  • Demonstrated computer skills
  • Demonstrated presentation skills
  • Demonstrated ability to work collaboratively and develop positive interactions with interdisciplinary members (physicians, staff and other disciplines) and patients/families/caregivers
  • Ability to communicate well, verbally and in writing
  • Ability to set priorities, achieve objectives, solve problems, meet deadlines, coordinate simultaneous projects, and make decisions

Proficient (or ability to become proficient) with computer operating systems including database, spreadsheet and word processing programs. Ability to design and run queries and produce reports

LICENSE/REGISTRATION/CERTIFICATION*

  • Current valid California RN license
  • Basic Life Support (BLS) required at hire
  • Certified Diabetes Care and Education Specialist (CDCES) (previously Certified Diabetes Educator CDE) certification required within 90 days of hire.
  • If new graduate RN, CDCES certificate to be completed based upon timeline from Certification Board for Diabetes Care and Education

Works with members of the diabetes care program to maintain compliance with Joint Commission Disease Specific Standards for inpatient diabetes certification Performs concurrent and retrospective clinical data abstraction from patient records Works with diabetes program manager, diabetes coordinator, physicians/providers, nurses, IT, and other support staff to develop and evaluate opportunities Facilitates communication(s) to the interdisciplinary care team, including but not limited to community clinic(s), primary providers and/or endocrinologist(s), as well as the inpatient interdisciplinary team when working inpatient. Assists in the development of instructional material for use in diabetes educational programs (includes objectives, content, editing, design, printing and distribution). Demonstrates clinical expertise in diabetes management by serving as a clinical expert, role model and clinical consultant to patients, families/care givers, physicians, patient care staff and other ancillary staff as appropriate. Partners with the Diabetes Care Program manager to prepare and update manuals, guidelines and other materials contributing to improving clinical performance in diabetes management, updating staff instruction and the maintenance of national, hospital and departmental standards. Provides diabetes self-management education to both outpatients and inpatients as needed.

Compensation

The posted pay range complies with applicable law and reflects what we reasonably expect to pay for this role. Individual pay is set by skills, experience, qualifications, and internal/market equity, consistent with MarinHealth’s compensation philosophy. Positions covered by collective bargaining agreements are governed by those agreements.
